Data Controller Application Form

Pursuant to Article 11 of the Personal Data Protection Law No. 6698 ("Law"), certain rights to make requests have been granted to data subjects whose personal data are processed. Pursuant to Article 13 of the Law, applications regarding these rights must be submitted in writing and in the manner specified in the relevant legislation to RUA Software ("Company") located at Mustafa Kemal Mahallesi, 2118 Cadde, No: 4a/36 Çankaya/ANKARA, which has the capacity of data controller, through the following methods.

In this context, if the application is desired to be made in writing, the following form must be filled out. The relevant form;

• With the personal application of the applicant,
• Through a notary,
• Via registered mail with return receipt,
• By signing with a qualified electronic signature and sending it to the Company's registered e-mail address,
• By sending through the e-mail address previously notified to the Company and registered in the Company's system,

Can be submitted to the Company. Pursuant to paragraph 2 of Article 13 of the Law, your request will be answered as soon as possible and at the latest within thirty days, depending on the nature of the request. In addition, during the evaluation of the relevant requests, additional information may be requested from the applicant for the purpose of confirming the application and ensuring personal data security. If the response to the application requires an additional cost, the fees in the tariff determined by the Personal Data Protection Authority may be requested.

• I want to receive it in person. (In case of receipt by proxy, a notarized power of attorney or authority document is required.)

The information above is collected in accordance with the Personal Data Protection Law No. 6698 and the Communiqué on the Procedures and Principles of Application to the Data Controller, and will only be used for the purpose of responding to the applicant's request.

The Company does not accept any responsibility in case the information related to your requests that you submit within the scope of the form is not accurate and up-to-date or in case of unauthorized application, and all responsibility arising from illegal, misleading or incorrect application belongs to the applicant.
